Chronic Disease Management

Daily case management as needed for students with long term health concerns such as asthma, diabetes, hypertension, attention deficit disorder, cerebral palsy, hemophilia, seizure disorders.  The school nurse acts as a liaison between parents, other health care providers and the school, and interprets medical data for the school as needed.  Student health care plans are completed on an individual basis for students with chronic illnesses.
